2. Notice & Proof of Claim DBL Form for Short Term Disability

Part A – Claimants Statement (EMPLOYEE)
Part B – Health Care Provider’s Statement (PHYSICIAN)
Part C – Employer’s Statement (BENEFITS)

3. Certification for Health Care Provider for FMLA

Section I: To be completed by BENEFITS

Terms to Know


Family Medical Leave Act (FMLA)
12 weeks of job protection to care for a new child with no threat of job loss. This is available to all employees employed with Havas for at least one year.

Short Term Disability (STD)
Leave due to injury or illness. STD paperwork is filed for parental leave just in case you eventually need to extend your disability to long term.

Paid Time Off (PTO) 
The amount of vacation and personal time you have accrued.

Qualifying Life Event
The action for adding new dependent(s) to your existing coverage or for terminating coverage with Havas should you have new coverage elsewhere.
